Redefining outcomes in immune TTP: an international working group consensus report

چکیده
Abstract Immune-mediated thrombotic thrombocytopenic purpura (iTTP) is a potentially fatal microangiopathy caused by autoantibody-mediated severe deficiency of ADAMTS13. Standardized definitions response, exacerbation, remission, and relapse were initially proposed in 2003 modified the International Working Group for TTP 2017. These definitions, which have been widely used clinical practice research, are based primarily on platelet count benchmarked against timing discontinuation therapeutic plasma exchange (TPE). They do not incorporate ADAMTS13 activity or temporizing effects caplacizumab, novel anti–von Willebrand factor (VWF) nanobody. In light these limitations, IWG aimed to develop revised consensus outcome that anti-VWF therapy, using an estimate-talk-estimate approach. The updated distinguish remission (defined count) from activity). exacerbation only TPE but also therapy. Retrospective validation described, although they yet be prospectively validated. Clinical implications discussed example their application provided highlight relevance.

Standardization of bleeding assessment in immune thrombocytopenia: report from the International Working Group.
In a previous publication on new terminology, definitions, and outcome criteria for immune thrombocytopenia (ITP), the International Working Group (IWG) on ITP acknowledged that response to treatment should consist of clinically meaningful end points such as bleeding manifestations and that platelet count may not be the ideal parameter for capturing the benefits of therapy. The IWG now proposes...
متن کاملGeriatric assessment predicts survival and toxicities in elderly myeloma patients: an International Myeloma Working Group report.
We conducted a pooled analysis of 869 individual newly diagnosed elderly patient data from 3 prospective trials. At diagnosis, a geriatric assessment had been performed. An additive scoring system (range 0-5), based on age, comorbidities, and cognitive and physical conditions, was developed to identify 3 groups: fit (score = 0, 39%), intermediate fitness (score = 1, 31%), and frail (score ≥2, 3...

متن کاملInternational Working Group consensus response evaluation criteria in lymphoma (RECIL 2017)
In recent years, the number of approved and investigational agents that can be safely administered for the treatment of lymphoma patients for a prolonged period of time has substantially increased. Many of these novel agents are evaluated in early-phase clinical trials in patients with a wide range of malignancies, including solid tumors and lymphoma. Furthermore, with the advances in genome se...
متن کاملClassification, diagnosis, and approach to treatment for angioedema: consensus report from the Hereditary Angioedema International Working Group.
Angioedema is defined as localized and self-limiting edema of the subcutaneous and submucosal tissue, due to a temporary increase in vascular permeability caused by the release of vasoactive mediator(s). When angioedema recurs without significant wheals, the patient should be diagnosed to have angioedema as a distinct disease. In the absence of accepted classification, different types of angioe...
